We are seeking an energetic and innovative individual for the position of Care Manager. This position will focus on the Client care experience and customer satisfaction for clients across GTA and Kitchener-Waterloo regions . The goal is to maximize operational effectiveness within a client-centred model of care delivery and ensure that the execution of our policies and procedures are in alignment with our mission, vision, goals and values. Job Purpose or Summary:

The Care Manager is a key point of contact for customers. The primary responsibility is ensuring the quality of ongoing care. The Care Manager will oversee all care plans, ensure proper oversight and represent the best care providers in the community.

Essential Duties and Responsibilities:

Communicate with potential customers during the intake process, explain our services, manage differentiators and assess staffing needs.

Conduct in-person assessments with potential clients, understand their needs, answer their questions and concerns, establish a relationship of trust with their family and make them clients.

Establish and monitor a care plan for each client to ensure the level and quality of agreed-upon care

Conduct regular home visits, managing our care plans and meeting the needs of our clients.

Ensure that the training of our caregivers is adequate and allows them to provide clients with care consistent with the plan of care.

Work with the staffing team to ensure excellence in the quality of services provided to clients and their continued satisfaction.

Oversee the changing needs of clients by ensuring that the care team applies changes to care plans.

Provide after-hours support to clients, referral partners and potential clients as needed.

Participate in the training and orientation of caregivers.

Support employee morale and oversee their processes through face-to-face telephone communications.

Ensure compliance and enforcement of Home Care policies, procedures and documentation.

Knowledge and Skill Requirements:

Vulnerable Sector Police & TB Screening

A bachelor's degree in social work, gerontology, health or a related field is preferred

Two years of experience in social work, geriatrics or care management.

Experience demonstrating progressive leadership and a hands-on approach to monitoring and evaluating client care is preferred

Ability to liaise effectively and professionally with the client committee, including family, physicians, lawyers, etc.

The ability to work in a team.

Strong English language communication skills, both oral and written.
