The Indigenous Diabetes Health Circle’s (IDHC) Foot Care Program (FCP) seeks a well-organized and self-motivated person to provide fee-for-service contract work with the FCP. This Contractor will provide foot care services in the Hamilton and Brantford area for IDHC FCP clients every 6 to 8 weeks.

Agreement for Services : IDHC will engage with the Contractor to provide services on an independent Contractor basis. Neither the Contractor nor any present or future employee, servant or agent of the Contractor shall be deemed an employee, servant or agent of IDHC, for any purpose whatsoever.

Summary of Contract : The Foot Care Nurse will work collectively with the FCP and its clinic Hosts to deliver diabetic foot care services. This contract is open concurrently for three locations: Hamilton Regional Indigenous Friendship Centre, Iroquois Lodge and Ancestral Voices in Ohsweken. The Foot Care Nurse will work closely and communicate with both the FCP and the clinic Hosts to ensure provision of best quality care.

Tasks of this Contract :

Manage diabetic foot care treatment for ongoing clinic every 6 to 8 weeks or per treatment plan

Work with Host to coordinate clinic dates and times

Skin, nail and wound assessment and treatment of FCP clients per approved treatment plan

Coordination of referrals to local chiropodist for further care if needed

Report to FCP the status of clients on a per client basis

Submit initial and annual assessments and updated client lists for all FCP clients per clinic

Submit complete invoices and treatment notes online for payment of service

Contract Qualifications :

Registered Nurse or Registered Practical Nurse with certification in Advanced Diabetic Foot Care from a reputable school or institution

Proof of Insurance (copy of Registered Nurses' Association of Ontario (RNAO) or Registered Practical Nurses’ Association of Ontario (RPNAO) certificate is acceptable)

Knowledge of the delivery of mainstream and Indigenous foot care services

Established and proven record as a dependable and competent person, with strong communication and interpersonal skills

Co-operative, conscientious and reliable, with strong skills in planning and work organization

Ability to work independently with a minimal supervision

Experience in electronic report submission

Valid driver’s licence and reliable transportation

Cultural safety training is an asset
